As the certificates are indexed in numerical order by certificate number, I will require the certificate number of the deceased. To aid confirmation of obtaining the correct certificate, please provide the name of the deceased and date of death. If needed, I will attempt to locate the certificate number when provided with the pertinent details. If I am unable to find the requested document, then the discounted, “Document Not Found” fee of $7.50 will apply.
