FAQ for this lookup

It doesn't give the father's name, just the child's full name and the mother's maiden surname. Children given up for adoption DO appear in this index under their original index entry, giving the child's name given at birth and the birthmother's surname. For a particular birth date, place and gender I can give you a list of all matching index entries, and one of those will be the adoptee's original entry.

Oops... I think I accidentally just "ordered", but I meant to just ask a Question... Did you get my Question, abt IL search and SF search question? I have a man who was born in IL, who is adopted. I will get his exact dob, and pob and hope it was in Cook Co. I will get back in touch with you. Can I email you privately,,, can I get your Email address? Mine is: tinapeddie@yahoo.com - thank you. Where was the person born that you need the 1970 information birth cert for? That is, if you still need it.

I am in Illinois.
Emily, the petition to adopt notice would be helpful in that it would give his original name plus his adoptive father's name. But I went ahead and checked for you to be able to tell you if it is there, and his notice is not there. That is unfortunate.
However, there is good news, in that IL is now releasing original birth certificates of adoptees. If your grandfather is still living, he can request a copy of his. I assume he is no longer living though, and in this case any of his surviving children or his surviving spouse can request a copy. If you go to my webpage at www.jclaussen.net, then click on Adoption Search, you will find a link to the request form. It only costs $15 to request it. If they reply to you that they have no record of his original birth certificate that will be an indication to you that he was never legally adopted in the state of Illinois. Sometimes these arrangements took place without formalizing the adoption legally.
Another resource that might be helpful if you can locate his Soc. Security number, is to send off to the Social Security Administration for his SS application, long form. This should give his parents' names, including mother's maiden, and his exact date and place of birth, as well as other genealogical information.
