Chicago death certificates from 1878-1922 are online for free at FamilySearch's Record Search but if you've searched and can't find the record you're looking for there, I might be able to locate the record for you. I've searched for thousands of Chicago and Cook County death certificates over the past five years and sometimes that experience helps.

If found, I will provide you with a digital copy of the record. If not found, I will let you know how and what I searched. Turnaround time will range from a day or two up to a week depending on how challenging the search turns out to be.

Please note: Death certificate forms before 1910 did not ask for parent or informant names and forms before 1916 did not ask for spouse.

Yes, the undertaker's name is usually listed and I've only seen a few death records where a cemetery name isn't given.
The information will vary, depending on what an informant knew, but below is a list of information you might expect to find for the various years. 1878-1903 name sex and race age in years, months, days occupation date of death marital status nationality and place where born (usually a state or country) how long a resident in the state place of death (street address and ward) cause of death duration of disease place of burial name of undertaker 1904-1909 all of the information above father's birthplace (usually a state or country) mother's birthplace (usually a state or country) number of years in Chicago 1910-1915 all of the information above name and birthplace of father (usually a state or country) maiden name and birthplace of mother (usually a state or country) informant's name and address 1916 forward all of the information above name of spouse
You asked about death record lookups after 1915. I'm away this week and so I can't turn things around quickly so I took that offering down. I'll put it back up next week but, unfortunately, I dropped my microfilm scanner and it's broken beyond repair so I may be doing lookups paper copy to JPG for a while . . .
