Chicago death certificates from 1878-1922 are online for free at FamilySearch's Record Search but if you've searched and can't find the record you're looking for there, I might be able to locate the record for you. I've searched for thousands of Chicago and Cook County death certificates over the past five years and sometimes that experience helps.
If found, I will provide you with a digital copy of the record. If not found, I will let you know how and what I searched. Turnaround time will range from a day or two up to a week depending on how challenging the search turns out to be.
Please note: Death certificate forms before 1910 did not ask for parent or informant names and forms before 1916 did not ask for spouse.
Fee if I find the record you requested: $6.00
Fee if I don’t find the record after a thorough search: $2.00
Please provide the full index entry from the Illinois Statewide Death Index or tell me with the name and death or burial date (at least a year). It's also helpful to have an age and a last-known address, if available. If you need more than one record, please create a request for each one separately.
