These death certificates are for persons who are buried in Chicago but died elsewhere—outside the city, in another Illinois county, or out-of-state. The certificates appear to be on whatvever death certificate form was in use in Chicago in the year that the death occurred. Some of the certificates appear to be original—they are written in the quick handwriting that I usually see on certificates and most of the information is filled in—but many of them appear to have been filled in from other records, perhaps by the same person. They are in similar handwriting, provide minimal information, and have lines drawn through the spaces where no information was recorded.
