Request this lookup if you've checked for a Chicago/Cook birth certificate (1871-1922) at FamilySearch.org's Record Search with no luck and want to know if the birth was reported through delayed birth registration, probably in the 1940s. I will send you digital images of the pages that cover the surname you need.
Note: Because this index isn't alphabetical by given name, I cannot handle lookups for common surnames (there would be WAY too many images to make that practical) and in those cases I will decline the lookup and suggest that you order in the film at your local FHC so you can take a look.
