If you provide an index entry for a Chicago record from 1923-1947 from the Illinois Statewide Death Index, I will provide you with a digital image of the record.
If not found (rare, but it happens), I will let you know how and what I searched and there will be no charge except the $.50 Genlighten processing fee.
You can view a blank copy of the death certificate form used during those years from the link below. However, please remember that key information (like parent names) may not be there if the informant didn't know it.
Please note: Cook County death records up through 1922 are available online for free at FamilySearch Record Search!
Fee if I find the record you requested: $6.00
Fee if I don’t find the record after a thorough search: $0.00
If you copy and paste the full index entry without making any tweaks it's easier for me to do the lookup. (The index entry will look something like this: ACKLEY AGNES F/W UNK 6016485 1931-05-31 COOK CHICAGO) If you can't copy and paste, please carefully type the name, death date, city of death and certificate number.
If you need me to look for more than one record, please create a separate request for each one.
