Chicago Death Index ("Burial Permit Index"), 1871-1933 from chicagogenealogy

I can provide scanned images from the Chicago Death Index, 1871-1933.

In most cases this index overlaps the online Illinois Statewide Death Indexes that you find online but it includes some entries that you won't find there and it's a "next step" if you can't seem to find a name in the online index. It includes deaths before 1878, out-of-town deaths, and stillbirths. Also, because it includes address of death, it can be very helpful in distinguishing between many individuals of the same name.

The index is arranged by surname. If the surname isn't common, I will do my best to get you all the entries. If it is common, I will only scan the pages with the first name you asked for. (If you need all the entries for a common surname, please submit a custom lookup request.)

I will charge the "found" price as long as I am able to locate a page where a name appears or where it would appear if it was there.

Fee if I find the record you requested: $5.00

Fee if I don’t find the record after a thorough search: $0.00

FAQ for this lookup

No. Even though this index is often referred to as the "Burial Permit Index" (probably because it includes out-of-town deaths), it actually seems to lead to "regular" death certificates. In fact, from 1916 forward, the numbers in this index are the same as the numbers you'd find in the Illinois Statewide Death Index,
