Chicago Death Certificate, 1916-1947 from chicagogenealogy

If you provide an index entry for a Chicago record from 1916-1947 from the Illinois Statewide Death Index, I will provide you with a digital image of the record.

I can locate Chicago death certificates using microfilm available at my local Family History Center when you provide index information from the Illinois Statewide Death Index, 1916-1950.

If found, I will provide you with an image scanned directly from the film. I will also provide you with a second image with the brightness/contrast tweaked, as need, to make the record as clear and readable as possible.

If not found (rare, but it happens), I will let you know how and what I searched.

You can view a blank copy of the death certificate form used during those years from the link below. However, please remember that key information (like parent names) may not be there if the informant didn't know it.
