I can locate Chicago death certificates using microfilm available at my local Family History Center when you provide index information from the Illinois Statewide Death Index, Pre-1916.
If found, I will provide you with an image scanned directly from the film. I will also provide you with a second image with the brightness/contrast tweaked, as need, to make the record as clear and readable as possible.
If not found (rare, but it happens), I will let you know how and what I searched. Turnaround time: Usually a few days, sometimes quicker. Please note: Death certificate forms before 1910 did not ask for parent or informant names and forms before 1916 did not ask for spouse.
