Chicago Death Certificate (you don't have a certificate number), 1878-1947 from chicagogenealogy

Request this lookup if you haven't been able to find the death record you're looking for in any of the indexes available online. I've searched for thousands of Chicago and Cook County death certificates over the past five years and sometimes that experience helps. I also have access to resources that aren't online including the Chicago Death Index, 1871-1933 and certificate films.

If found, I will provide you with a digital copy of the record. If not found, I will let you know how and what I searched. Turnaround time will range from a day or two up to a week depending on how challenging the search turns out to be.

Please note: Death certificate forms before 1910 did not ask for parent or informant names and forms before 1916 did not ask for spouse.
