Chicago Death Certificate--Can't Find the Name in an Index, 1878-1947 from chicagogenealogy

Can't find your ancestor's name in the Illinois Statewide Death Index? I might be able to help!

I have over five years' experience locating these records I will do my best to find what you need even if you have not been able to find a matching index entry in the Illinois Statewide Death Index. If I'm successful, I'll provide you with a digital copy of the record scanned from microfilm available at my local Family History Center. If I'm not, I'll give you a detailed report of what and how I searched.

My search may include one or more of the following: searching the online index to look for misspellings; checking the Chicago Death Index, 1871-1933 (on film); and scrolling through certificates for the right month/year.

Fee if I find the record you requested: $10.00

Fee if I don’t find the record after a thorough search: $5.00

FAQ for this lookup

No, but if I have a name, a last-known address, and an approximate death date, I can often pick out the match pretty quickly using the "Chicago Death Index, 1871-1933" (on film). A cooperative John Smith who didn't move very often and died at home in Chicago--piece of cake! <laugh> C

Ordering instructions from chicagogenealogy

Please give me the name of the deceased and a death or burial date. Age, place of birth, occupation, and last-known address can also be helpful. Parent names are helpful from 1910 forward.

If you're uncertain of the death date, please estimate and give me a quick summary of how you made it. Using census records, you may be able to narrow it to at least a ten-year period.

If you need me to look for more than one record, please create a separate request for each one.
